Dr. Morrison is an out of network provider. If you have out of network benefits for mental health and plan to submit to insurance, you can request a claim form to file for reimbursement.
By law, you have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost, if you do not have insurance or will not be submitting a claim to your insurance.
You have the right to receive a Good Faith Estimate for the total cost of any non-emergency services. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 day before your medical service. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. You will receive the Good Faith Estimate in the appointment confirmation email. Prior to the history-taking interview, you will receive a separate email that allows for electronic signature of the Client Information form and the Maryland Psychologist Agreement. The documents below are for your reference, and you may request electronic signature of the release of information form if you would like your evaluator to provide the report to other professionals. You may wish to bring copies of previous evaluations, grade cards, standardized testing results, or samples of school work/writing to the first appointment.
