Dr. Morrison is an out of network provider. If you have out of network benefits for mental health and plan to submit to insurance, you will receive a claim form to file for reimbursement.
Under a new law, you have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost, if you do not have insurance or will not be submitting a claim to your insurance.
You have the right to receive a Good Faith Estimate for the total cost of any non-emergency services. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 day before your medical service. You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. You will receive the Client Information form below in an email prior to the history-taking interview. You will separately receive for electronic signature the Good Faith Estimate (if appicable) and the Maryland Psychologist Agreement. The documents below are for your reference, and you may use the release of information if you would like Dr. Morrison to provide the report to other professionals. You may wish to bring copies of previous evaluations, grade cards, standardized testing results, or samples of school work/writing to the first appointment.
